Schedule
The typical day begins in the morning with endorsements from the night float team...no chief resident or faculty is present and morning report is for namesake only...
Attending rounds are highly erratic from anytime in the morning to up to 3-4 pm...mostly meaningless unless service attendings...noon conference at 12...mediocre and which many miss anyway due to too much patients and abundance of scut work...do your work in the afternoon...have clinic twice a week when you are on floors...average 80 hours /week...scut work is the killer in this program...anything from blood draws, blood gases, cultures, iv lines, taking patients down for radiological procedures...you name it , we have it!....no caps...ONCE ONE OF MY CO-INTERNS HAD 26 PATIENTS...FOR GODS SAKE!...they say they are trying to change it...but 15-20 patients per intern no uncommon...teaching, patient care, learing...everything gets compromised
Teaching
Faculty is average, not too keen on teaching except a couple...teaching is mediocre at its best...pvt attendings are concerned with covering their backs...often clueless...have to literally run and beg for discharges....noon conferences of some interest once in a while...the grand round of fridays is one bright spot...good talks from some established speakers...fellowships..one slot at SUNY in pulmonary sort of reserved...otherwise bleak...letters of recommendation are standard format for all and not personalised....nor any active interest taken by faculty to promote their residents
Atmosphere
Camaraderie among peers is good but limited as most are fmgs new to the country and in the process of settling down and getting secure...attending resident relationships are ok...no tension...mostly a stressed atmosphere with each intern running to finish his work...hardly any social get togethers...no such initiative taken by faculty either...recently residents voted unanimously for a union and things are improving...a brand new canteen with a meal allowance and palatable food...
Conclusion
Conclusions...if you are mediocre and want to drift through 3 years ....fine....second and third years are a piece of cake...you won't have much to cherish over 3 years though...once you finish, getting a job is no problem and I guess you will be ok...good fellowships..forget it...program it seems is making an effort(or so it claims) but the results no palpable as yet...
to conclude if h-1 visa is your priority and then primary care...its ok...everything else...not the place to be if you have a choice
